Please bring all your medical insurance cards with you to your appointment. Our staff will submit your insurance claims directly to your carrier. However, we will require you to sign an authorization for insurance payment. While we will assist in working with your insurance company, please remember that you are ultimately responsible for any charges. Co-payments and any patient-responsible balances are due at the time of service.
We accept Medicare assignments and participate in most major health insurance plans. Our business office staff can confirm our participation in your specific plan. Please bring those forms to your appointment if your insurance requires authorization or referral. Your Primary Care Physician may also fax the forms to our office at 614-221-5613
Payment is expected at the time of service for patients without insurance or those involved in motor vehicle accident claims.
