What to Bring

Completed pre-admission forms
Current insurance card(s) and a driver’s license
Any co-payments or deductibles (which are expected at the time of service)
If your insurance company requires a referral from your Primary Care Physician, please ensure this is obtained before your appointment.

If you’ve completed any relevant tests within the past year, please bring a copy of those results. If you cannot get the documents, please have the contact information for the facility where the tests were performed so we can retrieve them.
