Appointment Details
Your pre-admission testing appointment will take approximately 1-3 hours, depending on the specific tests ordered by your surgeon. Please plan accordingly.

What to Bring

  • Completed pre-admission forms
  • Current insurance card(s) and a driver’s license
  • Any co-payments or deductibles (which are expected at the time of service)
  • If your insurance company requires a referral from your Primary Care Physician, please ensure this is obtained before your appointment.

If you’ve completed any relevant tests within the past year, please bring a copy of those results. If you cannot get the documents, please have the contact information for the facility where the tests were performed so we can retrieve them.

Insurance and Billing 

We will assist you with filing claims with your insurance company. If you have questions regarding coverage, referrals, or co-payments, please contact us in advance to clarify any concerns.
